My consulting services include smoke inhalation chemical inhalation.
The issue of Ativan, Roxanol and Trazadone in COPD relates to their potential for depressed respiration and, in the case of Ativan, paradoxical agitation and confusion.
If a patient has severe COPD and has mutually accepted comfort care or hospice care then those medications can be appropriately provided for comfort.
However, if the patient desires aggressive care then the issue is one of risks v benefits, informed consent, educating the care givers for potential side effects and close monitoring.
All of this is clearly within the purview of any pulmonary specialists and is dealt with very commonly. Since COPD is so common and severely ill patients are frequently seen by pulmonary specialists this scenario is not unusual and is commonly dealt with by lung specialists.
